By scheduling or participating in an ultrasound session with Southern Mommies 4D Peek Boutique, you acknowledge and agree to the following terms:

• All ultrasound sessions provided by Southern Mommies 4D Peek Boutique are elective and non-diagnostic.
• These sessions are intended solely for entertainment, bonding, and keepsake purposes and should not be used as a substitute for medical care, testing, or diagnostic evaluation by your healthcare provider.

• All ultrasounds are performed by registered OB/GYN sonographers who are highly trained in imaging and patient care.
• Despite their credentials, the services provided through Southern Mommies 4D Peek Boutique are not medical examinations and do not include medical interpretation, measurements, or diagnosis.

• Southern Mommies 4D Peek Boutique is not responsible or liable for any injury or harm, physical or otherwise, to the expectant mother or fetus that may occur during or after the session.
• If any potential abnormality or area of concern is incidentally noted during the session, it may be communicated to your OB/GYN for further medical evaluation.
• Clients are required to disclose any known pregnancy complications, abnormalities, or conditions (including high-risk factors or restrictions from your healthcare provider) before their appointment so that our staff may ensure a safe and appropriate experience.

By booking or attending a session, you acknowledge that:

• You are currently under the care of a licensed healthcare provider for prenatal care.
• You understand that this is not a medical ultrasound, and no diagnosis or measurements will be provided.
• You consent for Southern Mommies 4D Peek Boutique and its sonographers to conduct the ultrasound for keepsake and bonding purposes.
• Your consent is implied through scheduling and participation in our services.

• The ability to capture 4D images or determine gender is dependent on baby’s position, movement, and cooperation during the session.
• If we are unable to obtain clear 4D images or determine gender due to baby’s position, we gladly offer one complimentary reattempt, scheduled no sooner than 7 days after the original appointment.

• If you choose to hear your baby’s heartbeat before 13 weeks, please note:
  • National ultrasound safety guidelines advise against the use of pulse-wave Doppler for measuring or listening to fetal heart rates in the first trimester.
  • For safety, we will use M-mode only for early heartbeat viewing.
  • If you specifically request Doppler before 13 weeks, it will be performed against recommendation and at your own risk, and Southern Mommies 4D Peek Boutique is not liable for any potential adverse outcomes.

• Images and video recordings from your session are for personal keepsake use only and are not intended for medical review.
• All images remain the property of Southern Mommies 4D Peek Boutique, and we may use non-identifiable images for marketing or educational purposes.

• By scheduling or participating in a session, you authorize Southern Mommies 4D Peek Boutique to contact your listed prenatal care provider if an incidental finding appears to warrant medical follow-up.
• You understand that such communication is offered as a courtesy only and does not constitute a medical diagnosis, interpretation, or evaluation.
• You acknowledge that Southern Mommies 4D Peek Boutique and its sonographers are not liable for:
  • Identifying or failing to identify any fetal, placental, or maternal abnormality;
  • Communicating or failing to communicate a potential concern;
  • Any outcome related to your pregnancy, delivery, or medical care.
• You agree that all medical evaluation, diagnosis, and follow-up are the sole responsibility of your licensed healthcare provider.
